While we continue to advance our acute treatments for stroke, we will be challenged to develop new, innovative models of care to improve outcomes for those who survive their stroke. Many organizations already report new innovations in transitions of care and in stroke follow-up clinics after hospital discharge to address patient needs. Others employ stroke care navigators to help the patient and provider negotiate outpatient follow up. Remote patient monitoring is blossoming as innovative digital communication allows providers to monitor and manage chronic care in the outpatient care setting.
As this shift occurs, stroke centers will be increasingly challenged to think beyond the acute episode of care and contribute to new, and improved stroke systems of care.
This webinar will walk through why stroke centers are perfectly positioned to improve stroke survivor transitions of care and provide short- or long-term post-discharge outpatient care. Post-discharge care models will be presented including barriers to developing these models. Billing opportunities and large cost items will be discussed with the ultimate goal of building awareness of important considerations in providing post-discharge care. Both are critical to knowing how to talk with leadership to gain their buy-in and support.
